HOW DO I....

OBTAIN A REPEAT PRESCRIPTION?

If you are on regular medication and your Doctor feels that your medicines can be repeated without seeing you each time, you will be given a computer repeat prescription slip. For safety reasons we do not take requests for repeat prescriptions over the telephone.

You can either hand in your request at the reception desk, post, fax on 01395 223301 or via e-mail by completing the form below.

Please tick the medication you require and allow 48 hours for the prescription to be available. If you supply a stamped, addressed envelope we can return the prescription to you. Alternatively, all the chemists operate a service for receiving and collecting repeat prescriptions to and from the health centre. If you wish to use this service please indicate which chemist you want to use

Clarepharm Pharmacy is run by a consortium of Exmouth Pharmacies and enables patients, if they wish, to obtain their prescriptions on site.

Opening hours: Monday to Friday between 8.30 - 12.30pm and 2.00 - 6.00pm.
