At Claremont Medical Practice, in addition to ensuring our patients have the best access to care aiming at improving their physical health, we are determined to improve the mental health and well-being of not only our patients, but the wider community too.
Once a person has taken that brave step to ask for help, it is important that we can quickly identify the range of support services available and find the one most suited to that individual. Some of these might require a referral from a GP but many support services operate a “self-referral“ process. Meaning you can get started straight away.
